Physical Therapy

In-Network 

we are currently a provider for Medical Mutual and Anthem (Blue Cross Blue Shield). This means that we will bill your insurance directly for physical therapy services. Your physical therapy bill will be based on your insurers contracted rates with our business.

Out of Network

As an out-of-network provider, we do not bill your insurance directly. We offer affordable cash rates and will provide the necessary information to use your HSA/FSA or submit to your insurance for reimbursement through your out-of-network benefits. All payments are expected at the time of service.
