Do I need a referral for physical therapy?
No. In the state of Ohio, you may see a physical therapist without a referral for physical therapy. An assessment and plan of care may be provided to your physician upon your request. If, within 30 days, we do not see progress or are concerned that you need further evaluation, we will refer you to an appropriate provider.
A referral is never needed for a wellness or sports performance visit.
What should I expect on the first day?
We will review your medical history, current pain or injury, sports, activities, and training program. We will perform a comprehensive examination and an initial home exercise program. Wear comfortable clothing that will allow you to move easily. Wear shorts if we are examining your knee and/or a tank top if examining your shoulder.
How long are the appointments?
Plan to spend 50-55 minutes at each appointment.
How do I know if my insurance will reimburse me for my physical therapy visits?
Call the number on the back of your insurance card to ask about your out-of-network benefits for physical therapy and the requirements for reimbursement. We are happy to provide a form called a superbill to submit to your insurance. A few questions you may want to ask when you call about your insurance coverage:

What is your cancellation policy?
Prime Physical Therapy & Sports Performance is committed to providing exceptional, one-on-one care for our valued patients. We kindly ask you to call before 3:00 pm the day before your scheduled appointment if you need to cancel. A fee of $50 will be charged for an unnotified, missed appointment. Additionally, we ask you to arrive on time for your appointment so that we can give each patient undivided care. If you are late, your session may be shortened accordingly.
