Custom Orthotics: What They Are, Who Needs Them, and Why Prime Does Them Differently
If you've been dealing with foot pain, ankle instability, or recurring knee issues and nothing seems to fix it for good, custom orthotics might be the missing piece.
Prime offers custom orthotic fabrication as one of our services, and thfe way we do it differs from what you'd get at a podiatrist's office or a retail store. Here's everything you need to know.

What Are Custom Orthotics?
Custom orthotics are prescription foot devices that are built specifically for your foot based on a comprehensive clinical assessment of how your foot looks, how it moves, and what your body needs.
They sit inside your shoe and are designed to correct biomechanical issues, redistribute pressure, improve alignment, and reduce the forces that cause pain and injury over time.
The key word is custom. Not semi-custom. Not "fitted" off a shelf. Built from scratch for your foot, your gait, and your goals.
Who Can Benefit from Custom Orthotics?
Custom orthotics are particularly effective for:
Runners and active adults dealing with plantar fasciitis, shin splints, or chronic knee pain
Athletes who want to optimize how their body absorbs and transfers force during sport
People with flat feet or high arches who haven't found relief from store-bought solutions
Anyone with overpronation or supination that's contributing to lower leg or foot pain
Active adults on their feet all day who need better support for daily activities
Post-surgical patients returning to activity who need additional biomechanical support
If you've tried multiple insoles without lasting relief, that's one of the clearest signs that a custom solution is worth exploring.
What Makes Prime’s Approach Different?
Most places will assess your foot while you're sitting or standing still. They capture how your foot looks at rest and build from there. The problem is, that's only half the picture.
At Prime, we evaluate your foot in both static and dynamic positions, meaning we look at how your foot sits at rest AND how it actually moves when you walk. Those two things can tell very different stories, and missing the dynamic piece means missing critical information about what's really causing your pain.
Our full orthotic assessment includes:
Static alignment and anatomical assessment — your foot structure, arch height, and how your alignment affects the rest of your body
Mobile gait analysis — how your foot moves, loads, and pushes off during walking
Ankle motion and strength testing — identifying any mobility or strength deficits contributing to your symptoms
A three-dimensional foot impression captured in a weight-bearing position so the orthotic reflects how your foot functions under load
We then work with a local fabricator to build your orthotics to exact specifications rather than a generic mold modified to approximate your measurements.

Two Options Based On Your Needs
Custom Orthotics — $399
A full one-hour biomechanical assessment, followed by fabrication of a permanent custom orthotic. Assessment and orthotics included in the price. Orthotics are ready in approximately two weeks. This is best for adults and athletes with fully developed feet who want a precise, long-term solution.
Customizable Over-the-Counter Orthotics — $299
For patients who are still growing, we offer a heat-molded over-the-counter option. The same thorough assessment is conducted, and we customize a high-quality pre-made insert to your foot during your visit. These are ready the same day, and last up to two years. A more affordable entry point that's ideal for growing athletes with foot pain.

What About Insurance?
We will attempt to run your orthotic assessment through your insurance if you carry MMO or BCBS coverage. Coverage varies by plan. Some cover the orthotics, some cover the assessment, and some cover both. We recommend calling your insurance provider in advance if coverage is a priority, and we're always happy to help too.
Why Get Orthotics At A Physical Therapy Clinic?
A podiatrist can prescribe orthotics. A specialty shoe store can fit you for a higher-end insert. But a physical therapist brings something neither of those providers typically offers, which is a whole-body perspective on why your foot is causing problems in the first place.
At Prime, your foot doesn't exist in isolation. We look at how it connects to your ankle, your knee, your hip, and your overall movement pattern. If there's something else going on that's contributing to your foot pain, whether it’s a strength deficit, a mobility limitation, or a movement habit, we'll find it and address it as part of your care.
That's the difference between treating a symptom and solving a problem.
